Home and Community Care (HACC)

Things you need:

The HACC scheme is split into 2 areas. The old Gatton Shire falls under the Darling Downs health district, and the old Laidley Shire is under the West Moreton Health District.

If you live in the old Laidley Shire, you will need to contact the Ipswich Home Assist/Secure on 1300 656 207 for the HACC services available to you. The information below does not apply.

If you live in the old Gatton Shire, the Gatton Home Maintenance Service provides practical assistance with home maintenance and minor repairs for frail aged and young disabled people in the old Gatton Shire.

Your Coordinator will arrange for the work to be carried out by a Contractor.

What to expect?

Assistance will be given with home maintenance and minor repairs to allow you to live at home.

How do I get this service

We will not be able to provide this service without a referral from either the Rural Allied Health Team or the HACC Assessment Team. They will assess your condition and decide if you are suitable to join the service. The team will advise you and the service Coordinator of the decision. Should you be successful, then a referral will be forwarded to us.

Costs

The service receives limited HACC Funds each year, and prioritises this funding according to need.

All applications are assessed on level of need. Generally clients will be expected to pay for capital purchases plus an hourly service fee. In cases of extreme hardship service may be provided free of charge.

For mowing services, clients are required to pay 50% of the bill, and are limited to a maximum of 12 services a year.
